PARTICIPANT REGISTRATION FORM

RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT AND PARENTAL CONSENT

Please use the following waiver when you have any participant that is a minor

In consideration of participating in activities at Flying High Gymnastics, Inc, I represent that I understand the nature of these activities and that I am qualified, in good health, and in proper physical condition to participate in such activities. I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in the activity. I fully understand that these activities involve risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the "releasees" named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost and damages I incur as a result of my participation in these activities.

I hereby release and discharge Flying High Gymnastics, Inc., its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and owners and lessors of premises on which the Activity takes place (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses, or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the "releases" or otherwise, including negligent rescue operations and further agree that if, despite this release, waiver of liability, and assumption of risk, I, or anyone on my behalf, makes a claim against any of the Releases, I will indemnify, save and hold harmless each of the Releasees from any loss, liability, damage, or cost, which any may incur as the result of such claim.

I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law and agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

PARENTAL CONSENT
AND I, the minor's parent and or legal guardian, understand the nature of the above referenced activities and the Minor's experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby Release and discharge, AGREE TO INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from all liability, claims, demands, losses or damages on the minor's account caused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the above Releasees, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from any litigation expenses, attorney fees, loss, liability, damage or cost any Releasees may incur as the result of any such claim.

Flying High also has permission to use my child's picture in marketing publications without compensation to me.

Flying High provides a safe, clean, friendly and fun environment to host your child’s party. All of our parties include friendly, certified staff to assist throughout your party! We have several party packages to fit the needs of your family! Flying High birthday parties are ideal for children of ALL ages!

Deluxe Birthday Parties

 

  • These parties include your own private section of the gym as well as a private party room!
  • 18 children are included with all private parties! We also accommodate larger groups!

  • Our famous inflatable obstacle course is included with EVERY private party
  • Two trampolines are also included in the base price for additional bouncy fun!
  • A friendly and certified staff member is there for all of your party needs! Our staff works to make your party as safe, fun and convenient as possible…you can just sit back and enjoy it!
  • Your private birthday party attendant will provide games, obstacle courses and relay races upon request, at no extra charge!
  • Party guests will have a blast playing in our two foam pits!
  • 90 minutes of swinging, jumping and bouncing through the gym.
  • Each guest will receive a Flying High party favor and the birthday party child will receive a free Flying High t-shirt
  • We provide a private party room for the entire two-hour celebration.
  • Base rate: $279 for up to 18 children. Expecting more than 18 children? No problem. Please call for details.


Open Gym Parties

 

  • Open gym parties are easy and affordable! They run for two hours during our regularly scheduled open gyms. A private party room will be available for your use throughout the party.
  • Each guest will recieve a Flying High party favor and the birthday party child receives a free Flying High T-shirt!
  • Our Open Gym staff works to make sure all children are having fun while playing safely!
  • Open Gym parties include up to 12 children. We also accommodate larger groups!
  • Party guests will have a blast playing in our two foam pits!
  • All of our Open Gym staff members will be on hand to answer any questions you may have.
  • Our famous Inflatable Obstacle course is available along with 2 trampolines for additional bouncy fun.
  • Base Rate: $191 for up to 12 children. Expecting more than 12 children? No problem. Please call for details.


Tots Birthday Parties

 

  • Tots birthday parties are designed for children Kindergarten age and younger. They are available on select weekdays during little tykes time (tot open gym).
  • Your guests will have tons of fun on our famous soft, inflatable obstacle course and in the foam pit.
  • A private party room is available for the entire 90-minute party!
  • A certified staff member is on hand to supervise and ensure safety while the guests have a blast.
  • Our staff can also play age-appropriate games or set up obstacle courses or relay races.
  • The birthday child receives a Flying high Party Planet T-shirt to commemorate his or her special day!
  • Each party guest leaves with a fun Flying High party favor!
  • Base Rate: $140 for up to 12 children. Expecting more than 12 children? No problem. Please call for details.


Pizza Fun Night Parties

 

  • A party room will be available to you at no extra charge!
  • 3 hours of fun in the gym and party room!
  • Up to 12 children in the base price of the party!
  • Pizza and lemonade are provided for all children who attend the party!
  • Enjoy our FAMOUS SOFT INFLATABLE OBSTACLE COURSE and 2 foam pits!
  • Instructors provide supervision inside the gym.
    Our staff is also supervising Pizza Fun Night but can answer any questions you may have.
  • Try our TWO trampolines for additional bouncy fun!
  • Two HUGE foam pits!
  • Map/waiver inserts are available for your invitations. Please be sure to have each guest bring a signed waiver on the day of the party.
  • As a courtesy, printable invitations are available below.
  • Each of your guests will go home with a Flying High party favor and the Birthday Child receives a special FLYING HIGH T-SHIRT!
